Job Description:
A company is looking for a Complex Case Manager to deliver
long-term mental health care to high-acuity clients.
Key Responsibilities
Manage a dynamic caseload of high-acuity clients, providing ongoing
assessments and crisis interventions
Coordinate care with clients' support networks and healthcare
providers to enhance clinical quality
Research and negotiate community resources to address clients'
social determinants of health
Required Qualifications
Bachelor's Degree in Nursing required; preference for BSN and
Master's in a related field
At least 2 years of psychiatric RN experience, including crisis
management and risk assessment
Active RN Compact License required; CCM certification preferred
Experience with EMRs, Google Suite, Slack, Doxy.me, and
RingCentral
Ability to organize and prioritize tasks independently
